Conference Registration Information
May 8-9, 2008 SNOWBIRD CLIFF LODGE
REGESTRATION IS CLOSED
COST: The conference registration fee is $110.00 for two days or $65.00 for a one-day registration (registrations may not be shared). The registration includes lunches, continental breakfasts and evening social on Thursday. Parking is free, unless you choose to use valet parking.
HOW to register…Several changes, please follow instructions carefully!
If you are paying with a check or money order print out registration form, complete it and mail with money to: :

saved as a document, information filled in and then emailed back as an attachment)

Note: agencies with multiple registrations must complete at least one form (include additional names and contact information on separate sheet).
IMPORTANT: Agency, email and fax registrations will be accepted
ONLY if purchase order numbers are included.
Registration deadline is APRIL 11, 2008
Invoiced vouchers must be paid in full on or before May 2, 2008
Sorry- no refunds can be given unless a WRITTEN request is received on or before April 25th. A $10.00 processing fee will be subtracted from the refund. Name substitutions will be accepted until April 28th.
